HeRO Graft (Hemodialysis Reliable Outflow: uscita affidabile per emodialisi) è la sola soluzione d’accesso AV completamente sottocutanea clinicamente in grado di mantenere’accesso a lungo termine nei pazienti in emodialisi con stenosi venosa centrale. HeRO Graft è classificato dalla FDA statunitense come un graft , ma differisce da un graft AV convenzionale dal momento che non ha anastomosi venosa. Consiste di due componenti primari:

Un componente per impianto arterioso brevettato in ePTFE

Il componente per impianto arterioso HeRO Graft ha un diametro interno (ID) di 6mm, un diametro esterno (OD) di 7.4mm e una lunghezza di 53cm, connettore incluso . Consiste di un impianto per emodialisi in ePTFE con nervatura in PTFE per garantire una resistenza al piegamento in prossimità del connettore in titanio brevettato. Il connettore in titanio fissa il componente di impianto arterioso al componente di efflusso venoso. Il componente di impianto arterioso viene incannulato usando una tecnica standard secondo le linee guida KDOQI.

Un componente di efflusso venoso brevettato

Il componente di uscita venoso HeRO Graft ha un ID di 5mm, un OD 19F (6.3mm) ed è lungo 40 cm. Composto da silicone radio opaco con rinforzo in nitinol intrecciato (per la resistenza al piegamento e allo schiacciamento) e un marker radiopaco alla sua estremità distale.

Principali benefici

  • Meno infezioni: riduzione del tasso di infezioni del 69% rispetto ai cateteri. 1
  • Adeguamento superiore alla dialisi: 1.7 Kt/V: un miglioramento dal 16% al 32% rispetto ai cateteri. 1
  • Alti tassi di pervietà: fino all’87% di pervietà cumulativa 2 anni. 1,2

Indicazioni d’uso

Hero Graft è indicato per i pazienti in emodialisi con insufficienza renale cronica, che hanno esaurito tutte le vie di accesso periferiche. Consultare le “Instructions for Use” per indicazioni complete e controindicazioni. RX only.

HeRO Graft è classificato da FDA Graft Vascolare.

Procedura Chirurgica

1. Utilizzando tecnica Seldinger endovascolare, la Componente endovenosa viene posizionata nel terzo superiore dell’atrio destro

2. A livello del solco deltoido-pettorale, il connettore in titanio è collegato alla componente endovascolare.

3. Anastomosi standard tra graft in ePTFE e arteria target (>3mm).

Identificazione del paziente

Controllo e osservazione dell’accesso vascolare per dialisi:

  • Fallimento di AVF o AVG?
  • Il paziente è catetere dipendente o sta per diventarlo?
  • Il Kt/V attualemente misurato è minore di 1.4?
  • Il paziente presenta braccia gonfie, edema agli arti o vene collaterali prominenti sul torace?
  • Il paziente ha subito molti interventi (es, angioplastica)?
  • Il paziente registra una stenosi dei vasi centrali?
  • Nel caso di risposta affermativa ad una delle specifiche precedenti, si consiglia una Venografia Centrale per valutare lo stato della stenosi venosa centrale.

Valutazioni chirurgiche per Hero Graft:

  • Venografia bilaterale central per confermare la stenosi venosa centrale;
  • Mappatura dei vasi per confermare che il diametro dell’arteria sia ≥ 3mm
  • Trattamento medico per l’ipercoagulazione
  • Frazione di eiezione ≥ 20%
  • Pressione sistolica ≥ 100mmHg
  • Paziente libero da infezione
